Post-acute entities that interact with Baylor Scott & White discharging facilities can apply for a customized EpicCare Link access that allows them to accept and deliver secure communications electronically. EpicCare Link is a web-based portal that gives community providers and partners secure access to manage referrals, access patient information, place orders, and coordinate care.

epiccare link, EpicCare Link provides secure, read-only, real-time access to your patients’ full charts within Health First’s Epic data repository. This includes lab results, diagnostic reports, hospitalization records, procedural information, discharge instructions, medications, allergies, and medical history. A site represents your workplace and must exist before you can add user accounts. If someone at your workplace already has access, your workplace has a site.
